David Altchek, MD is an Associate Attending Physician at
the Hospital for Special Surgery and at the New York Presbyterian
Hospital. He is an Associate Professor of Orthopedic surgery at
the Weill Medical College of Cornell University. He is the Team
Physician for the U.S. Davis Cup Team and is the North American
Medical Director of the Association of Tennis Professionals. He
is also on the Medical Advisory Board of the New York State Athletic
Commission. Dr. Altchek graduated from Columbia College in 1978
with a BA and received his MD from Cornell University Medical College
in 1982. He completed both his residency and fellowship in Orthopedic
Sports Medicine at the Hospital for Special Surgery in 1988.

| Frank Cordasco, MD is an Assistant
Attending Surgeon at the Hospital for Special Surgery and
at the New York Presbyterian Hospital. He is also an Associate
Attending Surgeon at St. Luke's-Roosevelt Hospital Center
in New York. Dr. Cordasco is an Assistant Professor of Orthopedic
Surgery at the Weill Medical College of Cornell University
as well as an Adjunct Assistant Professor of Orthopedic Surgery
at Columbia College of Physicians and Surgeons. In 1979 he
graduated from the University of Arizona with a BS, Rutgers
University with his MS and UMDNJ-New Jersey Medical School
in 1985 with his MD. He completed his residency in orthopedic
surgery at Columbia Presbyterian Medical Center in 1989 and
stayed on there for his fellowship in Shoulder Surgery and
Sports Medicine.

Edward Craig, MD is an Attending Surgeon at the Hospital
for Special Surgery and is a Professor of Orthopedic Surgery at
Weill Medical College of Cornell University. He is also a Consultant
(Orthopedic Surgery) at Burke Rehabilitation Hospital and is an
Attending Surgeon in Orthopedics at St. Vincent's Hospital in Bridgeport,
CT. After receiving his BA in 1969 from Princeton University he
attended Columbia College of Physician and Surgeons where he received
his MD in 1973. Dr. Craig completed his residency in orthopedic
surgery at Columbia Presbyterian Hospital in 1980 and went on to
his fellowship in Shoulder and Elbow Surgery at Columbia followed
by a fellowship in Hand Surgery.

| Jo Hannafin, MD, PhD is an Associate
Attending Surgeon at the Hospital for Special Surgery and
at the New York Presbyterian Hospital. She is the Director
of the Women's Sports Medicine Center as well as the Associate
Director of the Laboratory for Soft Tissue Research, both
at the Hospital for Special Surgery. Dr. Hannafin graduated
from Brown University in 1977 with a BS in Aquatic Biology
and received her MD, MS and PhD (Physiology and Biophysics)
in 1985 from The Albert Einstein College of Medicine. She
completed her residency in orthopedic surgery in 1990 at the
Montifiore Medical Center and did her fellowship in Sports
Medicine at the Hospital for Special Surgery, which she completed
in 1992.

Chisa Hidaka,MD is an Assistant Scientist for
the Laboratory for Soft Tissue Research at HSS and she is the Principal
Investigator for the Institute for Sports Medicine Research. She
graduated from Barnard College with a BA in 1986 and received her
MD from Cornell University Medical College in 1994. Dr. Hidaka completed
her residency in orthopedic surgery at HSS in 1999 and also completed
a fellowship at the Belfer Gene Therapy Core Facility, Institute
for Genetic Medicine at the Weill Medical College of Cornell University
in 2000.

| John MacGillivray, MD is an Assistant
Attending Surgeon at the Hospital for Special Surgery and
the New York Presbyterian Hospital. He is also the Director
of the Sports Medicine and Shoulder Service at the New York
Hospital Center of Queens. Dr. MacGillivray graduated from
Bowdoin College in 1984 with a BA in Biology and received
his MD from Tufts University School of Medicine in 1990. He
completed his residency in 1995 at the Hospital for Special
Surgery and after a 3 -year tour of duty in the U.S. Navy
he returned to HSS for his fellowship in Sports Medicine,
which he finished in 1999.

Robert Marx, MD, FRCSC is an Assistant Attending Surgeon
at the Hospital for Special Surgery and is an Assistant Professor
at the Weill Medical College of Cornell University. He is the Director
of the HSS Center for Clinical Outcomes as well as being the Orthopedic
Director of the Sports Medicine Institute for Young athletes at
HSS. Dr. Marx obtained his BS in biology at the Universite de Montreal
in 1987 and received his MD from McGill University in Montreal in
1991. He also obtained an MS in Clinical Epidemiology from the University
of Toronto in 1996. He completed his orthopedic surgical residency
at the University of Toronto and his fellowship in Sports Medicine
at the Hospital for Special Surgery.
| Stephen O'Brien, MD, MBA is an
Associate Attending Surgeon at the Hospital for Special Surgery,
an Associate Attending Surgeon at the New York Presbyterian
Hospital and is an Associate Professor of Orthopedic Surgery
at the Weill College of Medicine of Cornell University. In
addition he is an Attending Surgeon at North Shore University
Hospital at Glen Cove, the Chief Orthopedic Consultant for
St. John's University and is the Medical Director of the New
York Racing Association. Dr. O'Brien received his BA from
Harvard University and his MD from the University of Virginia
School of Medicine in 1981. He recently received an MBA at
Columbia University. He took his orthopedic residency and
fellowship at the Hospital for Special Surgery which he completed
in 1987

Scott Rodeo, MD is an Assistant Attending Surgeon
at the Hospital for Special Surgery and the New York Presbyterian
Hospital. He is also an Associate Scientist in the Hospital for

Thomas Wickiewicz, MD is an Attending Surgeon
at the Hospital for Special Surgery and the New York Presbyterian
Hospital. He is a Professor of Clinical Orthopedics at Weill Medical
College and is an Orthopedic Consultant to St. Peter's College.
Dr. Wickiewicz is the Chief of the Sports Medicine and Shoulder
Service at the Hospital for Special Surgery. He graduated with a
BS (Cum Laude) from St. Peter's College in 1972 and received his
MD from New Jersey Medical College in 1976. Before starting his
residency, he completed a fellowship in Sports Medicine Research
at HSS, the completed his surgical residency in 1981. He was a Travelling
Fellow, Visiting Scholar in Sports Medicine from July through December
1982 and was a Fellow, Biomechanics Laboratory at the Southwest
Orthopedic Group in Inglewood, CA from January through June 1982.
